General Insurance Information

  • We do not take insurance directly, however your insurance company may directly reimburse you for our nutrition services if you have out-of-network benefits. Coaching sessions, meal plans and metabolic testing may also be able to be applied to your deductible. You can call your insurance provider ahead of time (see below for help) to see what you may qualify for regarding reimbursement and applications to deductibles. St. Petersburg Nutrition accepts health savings accounts (HSA) as well as flexible spending accounts (FSA).
  • You can ask insurance companies for an exception or a prior authorization if needed due to a lack of area providers – there usually are not any private practice Dietitians in-network believe it or not. We are happy to be an in-network provider so let your insurance company know!

HOW TO ASK YOUR INSURANCE COMPANY ABOUT YOUR COVERAGE:

Reference info:

St. Petersburg Nutrition National Provider Number (NPI): 1255815395

There may be two types of nutrition coverage with an insurance policy:

  1. Preventative Nutrition Counseling
  2. Medical Nutritional Therapy (MNT).

Your plan may offer one or the other or both.

STEPS:

  1. Call your insurance company using the number on the back of your card. Ask to speak to someone in Customer Service to VERIFY your benefits.
  2. Write down the date, time, person that you spoke with, and ask for a reference number.
  3. They will ask for the NPI (see above) for your provider.
  4. They will ask or give CPT (procedure) codes.  We use 97802 for an initial and 97803 for follow up nutrition visit. We use 94690 for metabolic testing. We use also use G0108 for individual diabetes education.
  5. They may ask for diagnosis codes.  Commonly used codes Z71.3 (prevention), E11.9 (type 2 diabetes), E10.9 (type 1 diabetes), E66.9 (obesity), K90.0 (celiac disease), N18.3 (chronic kidney disease).  *if you need help with the diagnosis code please contact us.
  6. Questions to ask:
    • Does my plan allow for Preventative Nutrition Counseling?
    • If Yes: How many visits allowed per year?
    • When does your calendar year start?
    • Do you need to have met your deductible before insurance coverage?
    • How much can be reimbursed?

If your plan DOES NOT cover Preventative Nutrition Counseling:

    • Does my plan have coverage for Medical Nutrition Therapy (MNT)
    • If Yes: What ICD-10 Codes are covered? (see diagnosis codes above)
    • How much can be reimbursed?
