Medicare may cover Medical Nutrition Therapy (MNT) ONLY if you have diabetes and/or kidney disease, AND your doctor refers you for this service (see referral form to download below). Your doctor MUST check one of the diagnoses codes listed on the nutrition referral form and a signature for Medicare to cover services. Your doctor needs to refer you for this service to be covered by Medicare.

You can present this referral form to your doctor for completion OR I can fax the referral form to your doctor with your request. Your doctor MUST check one of the diagnoses and codes listed on the Nutrition Referral form with a signature for Medicare to cover services. 

Who’s covered?
People who have Medicare and a diagnosis of:
  • Diabetes
  • Renal disease (people who have kidney disease, but aren’t on dialysis)
  • Have had a kidney transplant within the last 3 years

Your doctor needs to refer you for this service to be covered by Medicare.  Prior to your first appointment, you can present the Referral Form to your doctor for him/her to complete or you can call St. Petersburg Nutrition today and I will gladly reach out to your doctor and request a referral on your behalf.

How often is this covered?
Medicare covers 3 hours of one-on-one counseling services the first year, and 2 hours each year after that. If your condition, treatment, or diagnosis changes, you may be able to get more hours of treatment with updated doctor’s referral. A doctor must prescribe these services and renew your referral yearly if continuing treatment is needed into another calendar year.
